Amendments on documents

Amendments on My kid/birth cert document application. In pemilik pendaftar column, should be my name Grace Chai Kiao Siang &my mother's name Kew Moi Jin for both Emily Siaw Yun Xin &Kathleen Siaw Yue Qi. The address should be our current address.

Also amendment for RM1000 given by the government bank Islam form, should be me Grace Chai Kiao Siang be the pemohon & my mother's name Kew Moi Jin.

Also, don't include house accessment document in applying for new ic in change address column.
